Collection and Storage of Personal Information 


At Stef Quinton Counselling and Consulting, I am committed to collecting, using, storing, and disclosing client personal information responsibly and in compliance with my regulatory college (the OCSWSSW) and privacy laws (the Personal Health Information and Protection Act, 2004). I am required by law and by the OCSWSSW to retain personal information gathered during your time in therapy and store it securely for a specific period of time (in this case, ten years, as per the OCSWSSW's standards of practice) after which your file will be destroyed. I use a secure, web-based professional practice management system called Jane to store electronic client records. The information collected during the course of therapy is used solely for the purpose of providing professional and responsible psychotherapy.

Your Rights 


You have the right to access and view the personal information that I collect. You may also ask for any information to be corrected. Please contact me with questions and concerns.

Confidentiality and Limits to Confidentiality 


I do not share client information without a client's written consent. However, there are limits to confidentiality and circumstances that would require me to share client information with third parties without a client's consent. Any disclosure that is made as a result of those circumstances will be limited to information that is necessary while protecting personal details of a client's therapy treatment. The limits to client confidentiality are as follows:
  • If we believe there is a risk of serious bodily harm to yourself or to someone else. 
  • If there is a reason to believe a child is in an abusive or neglectful environment. 
  • If we hear that another regulated health professional is breaking professional ethical standards. 
  • If our records are subpoenaed (i.e. requested by a court of law) or audited (by the Ontario College of Social Workers and Social Service Workers).
